Form 3040 – Student Entry Health Exam (physical examination) less than 1 yr.
Form 680 or Exemption – Florida Certification of Immunization (shot record)
Child’s birth certificate (used for proof of age for the VPK Program and private students)

Road to Success Academy Learning Center Enrollment Form
Valid government I.D for parents and/or guardians.
Current school readiness voucher, VPK certificate, Private school award letter.
Copy of child's birth certificate (proof of age for VPK and private school students only)
One- Time Registration Fee (non-refundable)
First week Tuition Payment
